Who it is for
Patients newly prescribed a medicine for asthma, COPD, diabetes, blood pressure or blood thinning.
What happens at the appointment
- A first conversation when the medicine is dispensed
- A follow up call or visit at around two weeks
- A final review at four weeks
- Practical help with side effects and routines

Why does this matter?
Most people who stop a new medicine do so in the first month. A short conversation early on prevents that.
